• All student-athletes must complete all forms listed in the Student-Athlete Medical Information Packet, including a Pre-Participation Letter to the parents/student-athlete, Medical Policy forms, Insurance Information and Medical Statements.
• Please have the packet completed and returned with signatures from both a parent/legal guardian and the student-athlete to the A&M-Commerce Sports Medicine office two weeks prior to the start of participation/pre-participation physicals.
• It is important for all student-athletes to read over the entire packet.
TRYOUT INFORMATION
Tryout Medical Release Form (PDF)
In order to tryout for an athletic team at Texas A&M University-Commerce, the prospective athlete must have completed these four requirements ahead of time with verification brought with them to the scheduled tryout:
• A physical from a licensed physician clearing them for
athletic participation within the last six months.
• A sickle-cell solubility test results on file or the
corresponding waiver form available here.
• The NCAA Division II Drug Testing Consent form available
here.
• Show proof of good academic standing and enrollment as a
full-time student at A&M-Commerce. Verification can be shown by
presenting a copy of their current class schedule.
No student-athletes will be allowed to try out for any A&M-Commerce team unless all four of these requirements are met ahead of time.
Just participating in a tryout does not ensure a spot on a team at A&M-Commerce team. If the student-athlete is selected to be a member of the team, they will be contacted by the respective coach for further instructions.
Tryout dates are determined by the coaching staff of
the respective sport.

IMPORTANT NOTES
The sports medicine staff at Texas A&M-Commerce is dedicated
to preventing injury, rapidly diagnosing and treating injury, in
addition to maximizing athletic performance. Director of Sports
Medicine Dustan Thrift is assisted by full-time, cerified and
licensed athletic trainer Courtney Hobbs as well as certified and
licensed graduate assistant athletic trainers Tim Neal and Krystal
Tyree and 20 athletic training students.
The Lions largest treatment facility is located in the
A&M-Commerce Field House. At the Field House, available
equipment includes: cryotherapy, hydrotherapy, moist heat,
electrical muscle stimulation and ultrasound. A&M-Commerce
student-athletes also perform rehabilitation at the local Physical
Therapy Clinic.
